Apple tree not looking healthy

I've annotated the three earlier pictures.
I think the main current problem is scab.
I think there may be another problem brewing in the form of bitter pit.

One or two of the dark spots look slightly like codling moth damage but it's probably just a slightly unusual scab spot - and there's no sign of the brown sawdust/sand-grain like excrement coming out of a small hole.
