My mum has a clump of Belladona lillies in her border, each year they throw up half a dozen shoots but only half ever produce a flower.
Any ideas why this might be?

Have they been there a long time? I don't know about belladonna lillies in particular, but often when things get congested they flower less. You could lift and divide them - not sure when, but probably too late for spring division, maybe you can do them in autumn? -
Foxhillgardener could be right in that your Belladonna Lilies are over crowded. If you are going to split then do it after they have flowered and before they start to make new leaves. They like to be planted close together with the noses just poking out of the soil. Nerinies like the same treatment btw.Gardening requires a lot of water - most of it in the form of perspiration.
